There is the cam that would need to be changed so the valves will open and close correctly if you take for example a left turn engine and run it right the exhaust valve will open before the intake valve it will start and run but no power.

and the starter will need to spin left to right or right to left.

watter pump will need to be changed

Fuel timing will be the same.

That will get this running

I have done a cpl conversions for pulling tractors example 2 666 cumins along side each other one turning left one turning right to balance the tractor,
